Serving the Ryde, Hornsby, and Parramatta regions, we are committed to offering exceptional aged care services that empower older Australians to live independently, comfortably, and with dignity in their own homes and communities. We offer a broad range of tailored support, including Home delivered meals, Linen service, Medical Transport Service, Individual Social Support, Social groups and outings and more.

Anyone can become a carer. Carers come from all walks of life, cultural backgrounds and age groups. They may be spouses, parents, sons or daughters, siblings, relatives, friends or neighbours. Carers provide support to a family member or friend who has a disability, mental illness, chronic condition, terminal condition or is frail. CCA has a range of services to support carers such as:

Our counselling service offers support for clients as they move through changes in life and circumstances. Some people move through with ease, while others face difficulties and turmoil.

Counselling can assist to process your emotions privately and deal with your life situation better. Support is provided as you deal with the issues that are affecting you. Our counsellors can help you develop strategies and skills to manage your responses to your current situation.

There is no fee for this service. This service is offered to persons:

  • Aged 65 years and over (or 50 years and over for Aboriginal and Torres Strait Islander people);
  • At risk of premature or inappropriate admission to long term residential care;
  • A carer of someone in the above categories; or
  • NDIS participants.

CCA’s Food Services provides support to all eligible persons. This includes any person who is:

We offer linen service for eligible clients. No need for you to purchase, wash or change your linen – it will all be done for you!

Linen supplied:

  • 2 bed sheets
  • 2 pillowcases
  • 2 bath towels

Your bed will be made up for you. The used linen and towels will be taken away and laundered.

A door to door car service taking eligible clients to their medical and rehabilitation appointments. Other types of transport (e.g. to visit someone in a nursing home) may be possible subject to the availability of drivers.

All fees are one way and not inclusive of tolls and parking fees. The kilometres (rounded to the nearest KM) below are calculated from the pick up address to the destination address (based on Google Maps).

Bookings are required to be made at least 3 working days prior to the appointment*. Please pay the driver at arrival of the destination, even if return transport is booked, and allow enough time for the appointment as drivers may not be able to wait if you go over the designated time**.

* While all attempts are made to service each booking, no guarantee can be given until we have confirmed with you that we have a driver for you. If we are unable to accommodate your booking you will be informed in advance so that you can make alternate arrangements.

** A late fee of $10 will be added to the transport fee for anyone who is late for their transport booking and/or goes over their waiting time with a grace period of 15 minutes. We advise that you consider allowing extra waiting time for appointments.

CCA offers a range of shopping services to eligible clients.

Bus Shopping

  • Door to door transport.
  • Volunteer assistance is available.

One-on-one shopping with a volunteer

  • Eligible clients are assessed and matched with a suitable volunteer.
  • Door to door transport.
  • Service offered subject to volunteer availability.

Internet Shopping

  • Clients call CCA to give their grocery orders.
  • CCA places the order online with Coles or Woolworths. Coles or Woolworths charges a delivery fee for each order.
  • Coles or Woolworths delivers the order to the client’s door.

CCA has a range of social activities and social support programs for eligible clients including:

  • Telephone Linkage – every weekday or weekly phone calls to check all is well.
  • Monday Lunch Outing – fortnightly.
  • Tuesday Outing – monthly.
  • Chinese Lunch Group – weekly Wednesday.
  • Chinese Bus Outing – monthly.
  • Cuppa Club – weekly Thursday, small group outings to cafes for morning tea.
  • Friday Lunch Group – fortnightly.
  • Individual Social Support – door to door service providing individual assistance and company for client’s activities.

How to receive government funded services?

Before you can receive government funded services, you’ll need to register with My Aged Care, and complete a RAS or ACAT assessment. This will determine if you’re eligible for the Commonwealth Home Support Program (CHSP).

Our specialist team is here to help you at any stage – from providing advice to help you with your initial registration and assessments, to choosing the services you need – so you can contact us before, during or after you start this process.

Stage 1: Book your assessments

Call 1800 200 422 or visit myagedcare.gov.au. They’ll register your details and refer you to your local Aged Care Assessment Team (ACAT) or Regional Assessment Service (RAS).

Your local RAS or ACAT will contact you to book a face to face assessment at your home or hospital to determine your needs and eligibility for government funding.

Stage 2: Book home care service

If you’ve been approved for the CHSP by the RAS or ACAT, you can receive funding straight away for some home care services.

Once you’ve completed your assessment, your assessor can refer you to us or give you your referral code. Contact us to find out how we can support you.

Stage 3: Get your CHSP and choose provider/s

We’ll come to you to discuss your needs, and answer any questions you may have. Once you’re ready and agree to services, our staff will work with you to develop a personalised care plan.

We’ll start providing your home care services. Our staff will check in with you each month to see if your needs have changed, and adjust your services if required.
